New S5. Add contact button takes me directly to Save Contact screen?

I don't get a screen that shows fields for info entry.. I have tried: Nothing else to try. I know of no other way to contact entry screen.

How to remove information in the notes section of a contact?

On one of the contacts on my phone I have clicked on notes and added information. Now I need to remove that info. How do I do it? I do not see how to remove the information, only how to add it.
