Resetting your phone causes this issue. When you perform a Factory Reset, you reset all of your settings to the factory default. To fix this issue, the best course is to contact your service provider and ask them to send you their network setup. They will send you the setup by SMS message. When you receive the message, you'll need to tap on it, which will and automatically add the APN to your phone. Once this is done, you won't have this problem anymore.
