The easiest way to do this is to simply call the US Consulate, and schedule an appointment with the consular officer. You'll discuss your visa options at that time, and they will tell you exactly what they expect from you. Remember that America is not like Jamaica, and they will process your request in person much more quickly than they will if you send a letter.

That said, if you prefer to send a letter, remember that any time you deal with the United States Embassy or US Consulate, the contents of your letter are always far more important than the actual writing. Any omission of fact, or misrepresentation of information, will result in your application being denied, and may even result in your own visa being cancelled.

In your case, you will need the following documents from your doctor, preferably notarized in the US (though this is optional). As they are going to the US Consulate, they will not need an Apostille. However, when you receive them, do not alter the documents in any way (no putting holes in them to bind them, or stapling things together - leave them in exactly the state they were delivered to you).

What the US Embassy will be looking for is any indication that you are being dishonest, or misrepresenting fact. If they determine that you are making a request without being truthful, or leaving out relevant details, they will deny your application for a visa to America.
