Is my modem hacked and my network compromised? If so what are the things hackers can see that I am doing on my computer?

I have Broadband Internet connection and I suspect that my modem has been hacked. Previously when I checked my IP address at various websites, they used to show the right external IP address provided to my computer by my ISP. But yesterday when I checked my external IP address on those same websites, everyone of them were showing totally different external IP address of my computer. They all were also showing the name of my ISP totally different and wrong. Also they were showing my location wrong. I am suspecting that my modem has been hacked and my network has been compromised. If so what are the things hackers can see that I am doing on my computer? How can I resolved this issue? I am using Windows 7 Home Premium 64 bit. Please Help.

Devices that connect to your computer do not reveal the names of their owners. IP addresses do not store any user's information either. The most information you can get is by entering the address into the input boxes on http://whois.arin.net/ui/, http://whois.domaintools.com/, https://www.maxmind.com/en/geoip-demo, and http://webhosting.info/whois/. You can also open your browser and enter the IP address to see if the person has a web page running.

Has 192.168.16.2 to 254. the other device has 192.168.1.2 to 254. I have tried: Manually change the second to fit the normal range but after some time it goes back
