Please provide a formal way of drafting a meeting minutes for the weekly team meeting

Date and Time of the meeting and those that attended the meeting go at the top. You then list the reason for the meeting and make an outline of things discussed. If just one person is chair or the only one speaking then you only have to list their name but if others speak during the discussion to make important points you will note their name and a brief description of what was discussed. Who was assigned to what should be noted.
