Correct way to write a motion letter to the judge?

I have to request for trial for two traffic citations be heard, I am past the 120 day deadline. However it is not intentional and there were several hiccups on the courts end. I was looking for sample to see the best way to approach it.

a. the accident took place 10/15/14 We were sited on 4/16/15 and his license was suspended the same day b. The ticket states that it has to be mailed in within 30 days. I mailed it on 5/12/15 but it was not posted that I requested a hearing until 5/19/15 C. when reviewing the ticket in the system it states that I have mailed requesting so I was patiently waiting. Come to find out it was never processed
